RACI Chart
Definition
A responsibility matrix defining who is Responsible, Accountable, Consulted, and Informed for each task.
A RACI Chart is a Step 4 (Select & Plan) tool that assigns clear ownership for every task in the implementation plan. R = Responsible (does the work), A = Accountable (owns the outcome, exactly one per task), C = Consulted (provides input before the decision), I = Informed (notified after the decision). It eliminates the ambiguity that kills projects: "I thought you were doing that."
