Lessons Learned
Definition
A structured document capturing what went well, what did not, and what to do differently next time.
Lessons Learned is a Step 6 (Evaluate) deliverable that captures the team's reflections through a structured retrospective. Each lesson includes: the situation, what happened, what was learned, and the recommended action for the future. Lessons learned feed directly into the next PIPS cycle, creating the "Close The Loop" mechanism that compounds organizational learning over time.
